This patient exhibited two tumours with very different tumour genetics and behaviour. While the cerebellar glioblastoma had the canonical POLE mutation (Figure 3) and was ultrahypermutant, the temporal lesion had a phenotype mimicking adult type glioma with significantly lower mutational load. Since response to immune checkpoint inhibition relates to mutational burden, it is interesting to note that the IDH1-mutated tumour had lower mutational burden and did not respond to immunotherapy.
In conclusion, this is, so far, the first case of a patient affected by CMMRD with two concomitant
IDH1 wild-type and mutated high-grade gliomas. Although the use of immunotherapy might improve the outcome of these patients with hypermutant cancers, since synchronous tumours are common in CMMRD, one should consider each cancer separately as these may exhibit different initiating events, patterns of progression and response to therapy including immune checkpoint inhibition.
