We presented a case of biopsy-proven isolated PCNSL of anterior visual pathway that is a rare entity. In the absence of concurrent systemic lymphomatous involvement, diagnosis of isolated PCNSL arising from anterior visual pathway is difficult with clinical and radiologic manifestations. Biopsy is essential to establish the histopathology. As a biopsy technique, endoscopic endonasal transsphenoidal or transtuberculer approach does not require optic nerve excretion, provides direct access to the lesion, better visualization, illumination and lesser morbidity as compared to open procedures, endonasal endoscopic procedure should be considered instead of open surgery.
